He was wounded in late October 2023 while attempting to inspect his bombed home in northwest Gaza City. The injury caused severe abdominal damage and led to multiple organ failures. His condition further deteriorated in mid-November during the Israeli military siege on the Al-Shifa Medical Complex, where he was denied medical care for over two weeks due to the bombing of medicine warehouses and the arrest of medical staff. Despite being transferred to a hospital in southern Gaza, Al-Shafei’s health continued to decline until he passed away.
